Privacy Practices and HIPAA Notice
Vibrant You Counseling is committed to protecting the privacy of every visitor to this website and every client of the practice. This Privacy Policy describes how information is collected, used, and protected.
Website information collection
This website uses minimal analytics and does not set tracking cookies. The contact form collects the information you submit (name, email, phone if provided, mailing address, reason for inquiry, and any optional message). Form submissions are protected in transit and stored only as long as needed to respond.
HIPAA Notice of Privacy Practices
Clinical services provided by Meghan Cobham, MHC-LP, are subject to the Health Insurance Portability and Accountability Act (HIPAA). Protected Health Information (PHI) shared during clinical care is protected by HIPAA and by New York State mental health confidentiality law. The clinical record is kept separately from any website inquiry record.
Limits of confidentiality
There are a small number of legally mandated exceptions to clinical confidentiality, including:
- Risk of imminent harm to self or others
- Suspected abuse or neglect of a minor or vulnerable adult
- Court order from a court of competent jurisdiction
These exceptions are reviewed in detail with every client at intake.
Telehealth platform
Sessions are conducted on a HIPAA-compliant secure video platform. Session content is not recorded.
Your rights under HIPAA
You have the right to access and amend your clinical record, to receive an accounting of disclosures, to request restrictions on certain uses, and to file a complaint with the U.S. Department of Health and Human Services if you believe your privacy rights have been violated. Filing a complaint will not affect the care you receive.
